THOUSAND OAKS, Calif. (AP) — An overwhelmed 911 system, chaos and delays hampered the law enforcement response to the November 2018 mass shooting at the Borderline Bar & Grill in Thousands Oaks that left 12 innocent people dead and 23 injured, a report released Wednesday concluded.
