Cards represents the first, theory-driven, culturally-tailored Advance Care Planning tool for Chinese Americans

San Jose CA – February 16, 2021 – The Chinese American Coalition for Compassionate Care (CACCC), in collaboration with authors: Zhimeng Jia, MD and Richard E. Leiter, MD, MA, both of the Department of Psychosocial Oncology and Palliative Care, Dana-Farber Cancer Institute, MA; Hillary D. Lum, MD, PhD of the Division of Geriatric Medicine, University of Colorado School of Medicine, CO and Cynthia X. Pan, MD of the Division of Palliative Medicine and Geriatrics, NewYork-Presbyterian Queens, NY, is proud to announce the recent publication of Heart to Heart Cards: A Novel, Culturally Tailored, Community-Based Advance Care Planning Tool for Chinese Americans in the American Journal of Hospice and Palliative Medicine®. CACCC’s Heart to Heart cards represent the first, theory-driven, culturally-tailored Advance Care Planning (ACP) tool for Chinese Americans. More research is needed to establish its impact on ACP conversations and outcomes.

“The publication of this article is a watershed moment,” said Sandy Chen Stokes, founder and board member of CACCC. “It takes our proof of concept of the Heart to Heart cards and café into the next level.”

“Though it has taken more than 18 months to have our manuscript accepted for publication, it was quite fortuitous in how it all came together with our collaborating authors: Drs. Richard E. Leiter, Hillary D. Lum and Cynthia X. Pan, during a global pandemic,” said Zhimeng Jia, MD with the Department of Psychosocial Oncology and Palliative Care, Dana-Farber Cancer Institute. “We were all very impressed with the data CACCC had collected since the introduction of their Heart to Heart cards and the Heart to Heart® Café events with the Chinese American community from 2014-2019.”

The top three (3) Heart to Heart cards selected by survey respondents were:

  • “If I’m going to die anyway, I don’t want to be kept alive by machines.” (25.5%)
  • “I don’t want to be a burden to my family.” (21%)
  • “I don’t want to suffer.” (19.7%)

“This study shatters the myth that Chinese Americans do not wish to discuss advance directives. We are looking forward to conducting a research study at NewYork-Presbyterian Queens using CACCC’s ACP Heart to Heart cards for hospitalized Chinese American patients,” said Cynthia Pan, MD. “The goal is to study Chinese American patients' engagement in ACP, in starting meaningful end-of-life conversation, to provide patient-centered care.”

“CACCC will host two free virtual Heart to Heart Cafés: one in Mandarin and Cantonese and one in English,” said Shirley Pan, director of CACCC. “Participants will learn how to use the Heart to Heart cards as well as learn tips on how to start the conversation with their family members, loved ones and their physician.”

Saturday, March 13: 10am-12pm, PST (1pm-3pm, EST) conducted in Mandarin and Cantonese

Saturday, March 20: 10am-12pm, PDT (1pm-3pm, EDT) conducted in English

Each participant who completes a program evaluation at the conclusion of the event, will receive one (1) deck of Heart to Heart cards sponsored by AARP.
