Madison Nursing Home Fined for Violations

The facility was one of three statewide which were recently fined by the state Department of Public Health.

MADISON, CT — A Madison nursing home was among three Connecticut facilities recently fined by the state Department of Public Health for incidents relating to patient care, the Connecticut Health I-Team (C-HIT) reported.

Madison House was fined $2,310 after a pair of nurse's aides moved a resident by themselves on two separate occasions, despite orders that require two-person assistance. The resident sustained minor arm injuries and was hospitalized in February, C-HIT said.

Apple Rehab Farmington Valley in Plainville and Parkway Pavilion Health & Rehabilitation Center in Enfield were fined $2,140 and $1,530, respectively, for incidents which occurred in 2016 and early Jan. 2017, including one in which a patient eventually died, according to C-HIT.
