Please read the most recent update from Region Food Services Department regarding Summer Meal Options and Student Food Services Account Balances Request Form.

As we close out the 2020-21 school year, please see the options below for high school seniors
(12th grade students) who will be graduating or for any other student who will not be returning to
Region 15 for the 2021-22 school year:

Please download the reimbursement form, complete, and return to bbrasche@region15.org.

1. Transfers: Remaining funds can be transferred to a sibling’s account.

2. Refunds: Remaining funds can be refunded only if you do not have another student in
the district. Please note refunds must be requested within one year after a student has
left Region 15 schools.

3. Donations: Please consider donating the remaining funds to the Region 15 Food
Services Program.

To elect one of these options, please complete a Student Food Services Account Balance
Request Form.
As a friendly reminder, account balances for all other students will be automatically ‘rolled over’
to next school year. As always, we
appreciate your partnership to provide our
students with healthy meals. If at any time
