Each item on an inspection form has a point value ranging from 1 to 9 points, with violations deducting points from a best possible score of 100. Higher points are taken for items with higher risk to cause illness, while repeat violations take even more points.

Letter grades assigned are A for totals of 100 to 90 points, B for 89 to 80, C for 79 to 70, and U for less than 70. Restaurants graded C and lower have their violations listed.

2-1B: .03(5)(c) Employee observed wearing gloves handled raw beef then changed gloves and touched raw vegetables. Vegetables discarded. Employee took off gloves and washed hands before continuing food prep. Employees must wash hands and exposed portions of arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service articles, and: 1) after touching exposed body parts; 2) after using the restroom; 3) after coughing, sneezing, using tobacco, eating, or drinking; 4) after handling soiled equipment or utensils; 5) during food preparation, as often as needed to remove soil and contamination and to prevent cross contamination when changing tasks; 6) before putting on gloves; 7) after engaging in other activities that contaminate hands; 8) a second time when entering the kitchen from using the restroom.

2-2B: .03(5)(j)1&2 Employee water bottle observed stored with spices above prep table. Water bottle discarded and all surfaces sanitized at time of inspection. Employees shall consume food and drink only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas (unless drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee's hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles). A food employee may not use a utensil more than once to taste food that is to be sold or served. Employees shall consume food and drink only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas (unless drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee's hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles).

2-2D: .06(2)(o) Observed corn in handwashing sink at beginning of inspection. Observed employee rinse frozen fruit package in handwashing sink at time of inspection. Training provided to employees. Handwashing sink washed, rinsed and sanitized at time of inspection. A handwashing facility may not be used for purposes other than handwashing.

8-2B: .07(6)(b) Spray bottle of chemical observed stored near dish machine not labeled with common name of chemical. Spray bottle labeled as bleach at time of inspection. Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material.

12B: .03(5)(g) Observed multiple employees wearing bracelets and watches preparing smoothies. All employees removed watches and bracelets before continuing to work and prepare smoothies. Food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food (except for a plain ring such as a wedding band).

14A: .04(4)(k) Observed in use utensils stored in water at 79F. Utilized washed, rinsed and sanitized and placed in clean dry container at time of inspection. Training provided to employees. During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored: 1. in the food with their handles above the top of the food and the container; 2. in running water of sufficient velocity to flush particulates to the drain, if used with moist food such as ice cream or mashed potatoes; 3. in a clean, dry container; or 4. in a container of water if the water is maintained at a temperature of at least 135°F (57°C) and the container is cleaned at the required frequency.
