• Facility does not have Employee Health/Reporting Agreement in place and signed. A health policy must be in place and each employee must sign the Food Employee Reporting Agreement. This documentation must be available upon request. This must be corrected as this a repeat violation. Physical handouts were provided during this inspection.
• There is no policy/procedure in the event illness causes vomiting and/or diarrhea occurs at facility. There must be a cleanup procedure/policy in place and this must be available upon request. Physical handout of approved vomit/diarrhea (norovirus) cleanup provided during inspection. This is a repeat violation.
• Observed 3-compartment sink dispensing very concentrated sanitizing solution in 3-compartment sink. Sanitizer should be dispensed at concentration between 150 ppm Quat to 400 ppm Quat. Upon testing, solution had a reading over 500 ppm Quat. This must be corrected immediately. Continue to dilute solution in 3-compartment sink until dispensing system gets serviced.
• There are no test strips at facility. Must obtain test strips for 3-compartment sink.
• Facility uses Time as a Public Health Control (TPHC) for the pizza sauce. The facility does not have the documentation for the variance. Must work on obtaining variance.

• Facility does not have an available health policy listing symptoms and illnesses requiring reporting and requirements to return to work. Employee Reporting Agreement was provided via website during previous inspection. This knowledge and documentation is required and required to be signed and readily available upon request. Employee Reporting Agreement/Health Policy provided via physical handout. All employees must sign. This is a repeat violation.
• Facility does not have a procedure for vomit/diarrhea cleanup. A cleanup procedure must be in place. Physical handout of a cleanup procedure was provided. This is a repeat violation.
• Observed no working food thermometer in facility. There must be a working, accurate temperature measuring device (thermometer) in facility to ensure proper reheat and cook temperatures of time/temperature control for safety (TCS) foods such as chicken wings. Must replace batteries or obtain a new thermometer immediately.
• Observed a plumbing leak at 3-compartment sink. This is a repeat violation and this must be repaired. Also, one of the Reach-In Coolers (RIC)/prep-top units had a bucket of liquid underneath units condenser/fan/cooling element. This must be repaired and corrected in 30 days.
• This facility uses Time as a Public Health Control (TPHC) for the pizza sauce. The facility has the required documentation and letter approving the variance. Must keep this at facility and posted.
