Observations: Employee grabbed lemons with bare hand and placed it on glass to be served to customer. Mozzarella cheese and pepperoni received at temperature above 41F. Meat slicer stored clean and covered had old food debris on it. Plates, utensils and sauce dishes stored clean had old food debris on them. Ice machine has mold like build-up inside of ice bin.Multiple sauce bottles removed from bulk containers stored at prep line and dry storage shelving not labeled with the common name. Only bimetallic thermometer available in facility. A thermometer capable of taking temperatures of thin massed foods shall be provided and accessible at all times.Sanitizer bucket stored above pizza boxes. Dish washer had build-up inside of machine. Floors and walls had grease build-up, old food debris and soil accumulation. Two holes found in walls in storage area. Surface of tile at prep line beginning to flake away.
