Each item on an inspection form has a point value ranging from 1 to 9 points, with violations deducting points from a best possible score of 100. Higher points are taken for items with higher risk to cause illness, while repeat violations take even more points.

Letter grades assigned are A for totals of 100 to 90 points, B for 89 to 80, C for 79 to 70, and U for less than 70. Restaurants graded C and lower have their violations listed.

2-1C: .04(4)(a)1,2,3,4(i)-(iii) Employee was observed placing acorn jelly onto customers plates with her bare hands. // Food employees shall not contact exposed, ready-to-eat food with their bare hands and must use suitable utensils such as deli tissue, spatulas, tongs, single-use gloves, or dispensing equipment except for if the ready-to-eat food is to be cooked in the food service establishment to heat all parts of the food to a time/temperature of at least 145°F (63°C) or if it contains a raw animal food it must be heated to the minimum cook temperature of the raw animal food.
Corrective Actions: All employees put on gloves to further handle food. //

2-2D: .06(2)(o) Both hand sinks in the kitchen were found severely blocked by a chest freezer and large equipment for noodles. // A handwashing sink shall be maintained so that it is accessible at all times for employee use.
Corrective Actions: Equipment was moved during inspection. //

6-1C: .04(6)(d) Multiple TCS food items (see temperature log) were actively in the cooling process but were not cooling rapidly enough to reach 41F or below within 4 hours. // Cooked time/temperature control for safety food shall be cooled: 1) from 135°F (57°C) to 70°F (21°C) within 2 hours; and 2) from 70°F (21°C) to 41°F (5°C) or below within 4 hours (provided the food met the first limit).
Corrective Actions: Foods were discarded or placed in the freezer to rapidly cool down within the leftover time. //

6-1D: .04(6)(i) Acorn Jelly, Fish Cakes, and Japchae (noodles) were all said to be on time control but were not marked. // "When using Time Control (TPHC) for a maximum of 4 hours: 1) hot foods shall start at 135°F (57°C) or above and cold foods shall start at 41°F (5°C) or below; 2) the food items shall be labeled with a start and discard time, not to exceed 4 hours; 3) foods not consumed before the discard time shall be discarded; 4) when combining ingredients, the start/discard time for the foods shall reflect the start/discard time of the earliest ingredient placed on time control; 5) written procedures TPHC shall be created in advance and made available upon the Health Authorities request."
Corrective Actions: Acorn Jelly and Fish Cakes were properly marked with their start and discard times during inspection. //

6-2: .04(6)(g) No food items in the walk-in cooler or in prep coolers were observed with date marks. // Refrigerated, ready-to-eat time/temperature control for safety foods prepared and held in a food establishment for more than 24-hours shall be clearly marked to indicate the date of prep or the date by with the food shall be used/discarded for a maximum of 7 days. The day of preparation shall be counted as Day 1.
Corrective Actions: All foods that date could be determined were properly labeled during inspection. If date could not be determined, food was discarded. //

12C: .04(4)(m) 4+ wet wiping cloths were found stored on prep surfaces throughout the kitchen, upon entry into the facility. // Cloths in-use for wiping counters and other equipment surfaces shall be stored in sanitizer of the appropriate concentration in-between uses and laundered daily.
Corrective Actions: All wet wiping cloths were returned to their sanitizer buckets during inspection. //

Corrective Actions: Chicken and rice reheated to 165F and placed back on steam table at time of inspection.

8-2B: .07(6)(b) Spray bottle of sanitizer not labeled with common name of chemical. Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material.
Corrective Actions: Labeled at time of inspection.

10D: .04(4)(d) Bulk sugar removed from original packaging at bar not labeled with common name of food. Except for containers holding food that can be readily and unmistakably recognized, such as dry pasta, working containers holding food or food ingredients that are removed from their original packages for use in the food establishment, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar shall be clearly and legibly identified, in English, with the common name of the food.
Corrective Actions: Sugar labeled at time of inspection.

13A: .02(1)(d) Inspection report posted is from 11/8/18. Most recent inspection was conducted on 6/17/19. The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away.
Corrective Actions: Inspection report posted at time of inspection.

14A: .04(4)(k) Handle of scoop stored in sugar and in rice stored in contact with sugar and rice. During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored: 1. in the food with their handles above the top of the food and the container; 2. in running water of sufficient velocity to flush particulates to the drain, if used with moist food such as ice cream or mashed potatoes; 3. in a clean, dry container; or 4. in a container of water if the water is maintained at a temperature of at least 135°F (57°C) and the container is cleaned at the required frequency.
Corrective Actions: Scoops removed at time of inspection.

17C: .07(5)(a),(b) Missing/ broken tiles observed in kitchen. All physical facilities shall be maintained in good repair and shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. Must correct by 11/10/19 or permit may be suspended.

Corrective Actions: The person in charge showed up one hour later.

2-1B: .03(5)(c) Observe multiple employees not wash their hands after eating their food. 2. employee not washing their hands after handling raw foods (raw chicken) Employees must wash hands and exposed portions of arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service articles, and: 1) after touching exposed body parts; 2) after using the restroom; 3) after coughing, sneezing, using tobacco, eating, or drinking; 4) after handling soiled equipment or utensils; 5) during food preparation, as often as needed to remove soil and contamination and to prevent cross contamination when changing tasks; 6) before putting on gloves; 7) after engaging in other activities that contaminate hands; 8) a second time when entering the kitchen from using the restroom.
Corrective Actions: The employees washed their hand after being told by the person in charge

2-2A: .03(2)(m) Observed employees without an employee health policy. Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food.
Corrective Actions: The person in charge reviewed the policy and had the employee sign it.

2-2B: .03(5)(j)1&2 Observed employees eating on food prep area at the start of the inspection and eating while making food for customers. Employees shall consume food and drink only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas (unless drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee's hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles).
Corrective Actions: Employees stop eating after being instructed by the inspector.

4-2B: .05(6)(n) Observe sanitizer of bleach concentration in the three compartment that employee was using to sanitize clean containers with low concentration of bleach. A chemical sanitizer used in a sanitizing solution for a manual or mechanical operations must meet a specified minimum concentration (for chlorine, sanitizer is must be 50-100 ppm; for quat ammonia, it must be used according to the manufacturer’s specifications).
Corrective Actions: The sanitizer was drained out and remade to have the adequate amount of bleach and the containers were placed in the soap water to be rewashed

6-1A: .04(6)(f) Observed container of raw chicken sitting at room temperature for one hour. Time/temperature control for safety food shall be maintained at 41°F (5°C) or below.
Corrective Actions: The raw chicken was placed back into the walk in cooler.

6-1C: Large container of white rice and cooked shrimp stored in the walk in cooler did not drop down in temperature. Cooked time/temperature control for safety food shall be cooled: 1) from 135°F (57°C) to 70°F (21°C) within 2 hours; and 2) from 70°F (21°C) to 41°F (5°C) or below within 4 hours (provided the food met the first limit). 2ND REPEAT VIOLATION. A THIRD REPEAT WILL RESULT IN A PERMIT SUSPENSION.
Corrective Actions: The food was discarded

6-1D: .04(6)(i) Observed container of rice and chicken wings placed on time control that were not labeled with the start and discard time. "When using Time Control (TPHC) for a maximum of 4 hours: 1) hot foods shall start at 135°F (57°C) or above and cold foods shall start at 41°F (5°C) or below; 2) the food items shall be labeled with a start and discard time, not to exceed 4 hours; 3) foods not consumed before the discard time shall be discarded; 4) when combining ingredients, the start/discard time for the foods shall reflect the start/discard time of the earliest ingredient placed on time control; 5) written procedures TPHC shall be created in advance and made available upon the Health Authorities request."
Corrective Actions: The rice and chicken were labeled with the start and discard time.

6-2: .04(6)(g) Observed food (pans of chicken) that were cooked over 24 hours that did not have the date it was made. Refrigerated, ready-to-eat time/temperature control for safety foods prepared and held in a food establishment for more than 24-hours shall be clearly marked to indicate the date of prep or the date by with the food shall be used/discarded for a maximum of 7 days. The day of preparation shall be counted as Day 1.
Corrective Actions: The food were labeled with the dates they were made.

10D: .04(4)(d) Observed multiple sauce take from its original container that were not labeled with what it is. Except for containers holding food that can be readily and unmistakably recognized, such as dry pasta, working containers holding food or food ingredients that are removed from their original packages for use in the food establishment, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar shall be clearly and legibly identified, in English, with the common name of the food.
Corrective Actions: The sauces were labeled with its common name.

10D: .04(8)(b) Observed two containers of expired Tzatziki sauce that contains milk(10/27/2019). Prepackaged sandwiches, eggs, infant formula, shucked oysters, milk, and time/temperature control safety foods that are labeled as “keep refrigerated” and that are for sale or service to the consumer or used as an ingredient in other foods shall be immediately discarded and shall not be sold, served, or used after the manufacturer’s expiration date or the sell-by date.
Corrective Actions: The sauces was discarded.

13A: .02(1)(d) Observed old inspection report posted. A food service establishment inspection report addendum need not be displayed, but shall be made available by the food service establishment to the public upon request.
Corrective Actions: The posted in charge posted the new inspection report.

14B: .05(10)(e) 1,2,4 Observed containers stored as clean and stacked wet. Clean equipment and utensils shall be stored in a self-draining position that allows air drying and covered or inverted. 2ND REPEAT OF THIS CODE PROVISION A THIRD REPEAT WILL RESULT IN A PERMIT SUSPENSION.
Corrective Actions: The containers were place in the soap water to be rewashed.

14C: .05(10)(f) Observed facility reusing hot sauce containers as a working food container. Single-service and single-use articles may not be reused.
Corrective Actions: this was not corrected.

15C: .05(7)(d) Observed containers stored with the clean containers with the old labels still on them. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
Corrective Actions: The labels were taken off and the containers were place in the soap water to be rewashed.

17A: .07(5)(h) Observed the men's toilet left unclean. Plumbing fixtures such as handwashing sinks, toilets, and urinals shall be cleaned as often as necessary to keep them clean and maintained.
Corrective Actions: The toilet was cleaned.
