Violations: 4-2B, Violation of Code: [.05(6)(n) ] Sanitizing solution for dishwashing procedure not meeting adequate concentration. Quat sanitizer at dish sink not at adequate level. Facility was actively washing dishes during insepction. The quat level is measuring below 150 ppm. Manufacturer's label stated that the adequate quat level was 260 ppm for food contact surfaces. Please have service provider of quat come and calibrate the system. In the interim, please mix the sanitizer manually to the appropriate level. Santizer for the dish wash procedure must be maintained at the adequate concentration. Corrected On-Site. New Violation. 6-1A, Violation of Code: [.04(6)(f) ] Cold-held potentially hazardous foods not maintained below 41oF; no time controls/documentation in place. Several items in the walk-in cooler were being held above 41 degrees. Walk-in cooler holding an air temperature of 42-43 degrees at the time of the inspection which was during the lunch rush. Cooler can hold an air temperature of colder but cannot handle the busy times of opening and closing the door during busy lunch. 1/2 and 1/2 in the bar area was being held above 41 degrees (at 60 degrees). Bar cooler holding an air temperature of 60 degrees. Several foods on the prep line were being held above 41 degrees on the top of the container. Food in the middle and bottom of containers were below 41 degrees. Some foods are being overstacked in the prep line cooler. Foods appropriately discarded or cooled. This is a repeat violation from inspection on 11/20/13. This is the 2nd violation of code provision .04(6)(f). If this code provision is marked out on the next inspection, PERMIT MAY BE SUSPENDED. FACILITY WOULD BE REQUIRED TO CLOSE IMMEDIATELY AND COME INTO GWINNETT COUNTY ENVIRONMENTAL HEALTH FOR A COMPLIANCE CONFERENCE. A CORRECTIVE ACTION PLAN AND RISK CONTROL PLAN WOULD BE REQUIRED. PLEASE CAREFULLY MONITOR ALL FOOD TEMPS AND COOLERS. Corrected On-Site. Repeat Violation. 11C, Violation of Code: [.04(6)(c)] Potentially hazardous foods not thawed under an approved method. Meat being thawed in pan of water. Water was turned on in meat sink. Potentially hazardous foods must be thawed under running water. Corrected On-Site. New Violation. 12A, Violation of Code: [.04(4)(q) ] Food not shielded properly on bar display shelves. Facilty has an open bar to the outside. Some liquor/mixers on shelves have a lid that is sealed but many have the pour spout that is not covered. Bugs can easily fly into these containers. Please cover these containers with the cone cups that were on a few bottles already. All food must be shielded properly during storage. New Violation. 12A, Violation of Code: [.04(4)(q) ] Food not stored at least 6 inches above the floor. Liquor/mixers stored on floor in bar area and in refrigerated storage area. Please move these items so that they are not on the floor. Must store all food at least 6 inches above the floor. New Violation. 12A, Violation of Code: [.04(4)(q) ] Food being stored in walk-in freezer not protected from overhead contamination. Water dripping down from condenser in walk-in freezer and soaking boxes of food and then freezing. Food is not being properly protected. Please move food so that it is protected and repair walk-in freezer. New Violation. 12C, Violation of Code: [.04(4)(m) ] In-use, wet wiping cloths not stored in an approved sanitizing solution (100 ppm available chlorine or 300 ppm available Quat) Sanitizer level in wiping cloth buckets (all) were below the acceptable level to sanitize. Have quat sanitizer dispenser at the dish point checked to ensure it is dispensing at the appropriate level. New Violation. 15A, Violation of Code: [.05(6)(q) ] Walk-in cooler not holding an ambient temperature of 41 degrees or colder during peak lunch period; air temp was 44 degrees. Bar cooler holding potentially hazardous food not holding an air temperature of 41 degrees or below; air temp was 60 degrees.
All coolers containing potentially hazardous foods must be capable of holding an air temperature of 41 degrees and below at all times. Repair company was called and arrived during inspection to service the coolers. This is a repeat violation from inspection on 11/20/12. Repeat Violation. 15A, Violation of Code: [.05(6)(c) ] Microwave oven in disrepair. Bottom is rusted out and pitted. Microwave oven must be replaced so that it is easily cleanable/washable. Microwave ovens are considered a food contact surface and must be maintained in good repair and adjustment. New Violation. 15A, Violation of Code: [.05(6)(a) ] Wire dry baskets in disrepair with broken wires. Fry baskets must be replaced. Food contact surfaces must be maintained in good repair and adjustment. New Violation. 15A, Violation of Code: [.05(6)(a) ] Several containers/utensils/lids for food contact not maintained in good repair. Containers/utensils/lids chipped, broken and cracked. Please discard these items. Must maintain all equipment in good repair and adjustment. This is a repeat violation from inspection on 11/20/12. Repeat Violation. 15A, Violation of Code: [.05(6)(a) ] Walk-in freezer dripping water from the condenser and icing up on the floor and on food containers. The entire floor of walk-in cooler is a sheet of ice. Water dripping from top of freezer during inspection. Must keep equipment in good repair and adjustment. Please repair the walk-in freezer. New Violation. 16B, Violation of Code: [.06(2)(r) ] Facility has several plumbing issues. 1. Leak under handwash sink on prep line. Bucket under sink catching water. 2. Leak at the spray arm next to the dishwasher. 3. Leak at the mop sink. Please repair these leaks. Plumbing system must be maintained in good repair. New Violation. 18, Violation of Code: [.07(5)(k) ] Fruit flies/nats swarming around in outside bar area. Please have pest control company address this issue. Keep floors dry, liquor/mixers covered, floors clean, counters clean. Must control pests in any food service area. Bar was not open during the inspection but the garage doors were open so that bugs could get into the area. Advise to keep these doors closed when not operating this area. New Violation.
