Violations: 4-2B, Violation of Code: [.05(7)(a)1 ] Several plates for service and a large metal bowl were observed with food residue on the food contact surface. The items were removed for rewashing. Food contact surfaces must be clean to sight and touch.  Corrected On-Site.  New Violation. 6-1B, Violation of Code: [.04(6)(f) ] A container of chicken broth and a container of shrimp broth were observed on the stove with temperatures under 135°F. The foods were reheated above 165°F. Foods being held hot for safety must be maintained at 135°F or above.  Corrected On-Site.  New Violation. 6-2, Violation of Code: [.04(6)(g) ] Several potentially hazardous foods (special sauce, cooked sweet potato, yucca, plum drink) were stored in the coolers without date labels. The foods were labeled with the date of preparation. Potentially hazardous foods being kept in facility for more than 24 hrs must be date labeled to not exceed 7 days (including date of preparation).  Corrected On-Site.  New Violation. 10D, Violation of Code: [.04(4)(d) ] MSG and mayo placed into containers without proper labeling. The foods were labeled. When foods are that are not easily identifiable are removed from their original containers the containers must be labeled with the common food name.  Corrected On-Site.  New Violation. 10D, Violation of Code: [.04(3)(k) ] Mussels were observed in the deep freezer in a plastic ziplock bag. The items must remain in the original container. Shellfish must not be removed from original container in which they are received other than immediately before the sale or preparation for service.  New Violation. 11D, Violation of Code: [.05(2)(x) ] No thermometer was located in the reach in cooler in front of grill area. The Person in charge must place one in there. A thermomenter must be used to monitor the temperature of a refrigeration unit.  New Violation. 12B, Violation of Code: [.03(5)(i) ] Food preparation employees observed preparing food without the use of hair restraints. They put on hairnets. Employees must wear an effective and clean hair restraint when preparing foods.  Corrected On-Site.  New Violation. 12D, Violation of Code: [.04(4)(g) ] Frozen squid was observed being thawed in the vegetable sink. The person in charge was instructed to thaw meat in the three compartment sink. The vegetable sink must only be used for washing vegetables.  New Violation. 14D, Violation of Code: [.04(4)(n) ] Employee touched hair with gloved hands. She then touched handles of fry basket with contaminated glove. The gloves were discarded, and the handles were sanitized. Single-use gloves must be discarded when they become soiled or damaged, or when an interruption of task occurs.  Corrected On-Site.  New Violation. 15C, Violation of Code: [.05(7)(a)2,3 ] Several metal pans stored clean in dry storage had flour on the outside; food labels on the outside of pans remaining. The pans are to be rewashed. Non-food contact surfaces must be from accumulation of dirt, debris, and residues.  Corrected On-Site.  New Violation. 17C, Violation of Code: [.07(5)(a),(b) ] 1. The wall and floor in the dry storage area had food debris on them. 2. Discarded gloves were under the racks in dry storage. Theses surfaces must be cleaned. The physical faciliess must be cleaned as often as necessary to keep them clean and by methods that prevent contamination from foods.  New Violation. 18, Violation of Code: [.07(2)(m) ] 1. Neither the screen door or outer door has a self closer on it. 2. The mesh screen door has a hole at the bottom left corner. Self closers must be installed, and the screen repaired. Outer opening must be protected to prevent the entrances of pest.  New Violation.

Violations: 4-1A, Violation of Code: [.04(4)(c)1(i)-(iii),(v),(viii) ] In prep cooler, raw chicken stored behind chorizo and raw beef. Raw chicken moved to the front with chorizo and beef moved to the middle. Containers of raw chicken stored over ready-to-eat foods in the bottom section of the prep cooler. Items moved to the other side of the prep cooler and shielding placed between the top and bottom sections of the cooler. Keep all raw animal proteins stored in the order of the required cook temperatures.  Corrected On-Site.  New Violation. 6-1A, Violation of Code: [.04(6)(f) ] Salsa and roasted salsa in ice bath by the chips warmer were being held above 41 degrees. Person in charge stated that these items had been out less than 2 hours. Ice added to ice bath to better cover the sides of the salsa containers. Make certain all potentially-hazardous foods are held at 41 degrees or below.  Corrected On-Site.  New Violation. 6-1C, Violation of Code: [.04(6)(d) ] Several items on prep table not cooling rapidly enough to reach 70 degrees within 2 hours and 41 degrees within 6 hours. Items were stirred and the temperatures dropped considerably after stirring. Stir wet or thick items frequently while cooling and closely monitor cooling temps to confirm that foods are cooling rapidly enough.  Corrected On-Site.  New Violation.
