1-2A: .03(2)(a)-(o) Several violations occurred during today's inspection resulting in an unsatisfactory score due to an overall lack in active managerial control. Some violations were repeat violations. // It is the responsibility of the person in charge to ensure compliance with the food code by demonstrating active managerial control within their facility.
Corrective Actions: Ensure managers/supervisors are training and monitoring employees to have food safety occur at all points of food served. //
1-2A: .03(1)(a)-(c) The persons-in-charge (2x) were not knowledgeable about food safety as it pertains to the Georgia Food Code. // The person in charge must be able to demonstrate their food safety knowledge through full compliance of the food code with no violations, be a Certified Food Safety Manager, or be able to provide correct answers to food safey questions as they relate to the food service operation at hand.
Corrective Actions: The employees were educated during the inspection. //
2-1C: .04(4)(a)1,2,3,4(i)-(iii) Observed employee using bare hands to make sandwich with bare hands. // Food employees shall not contact exposed, ready-to-eat food with their bare hands and must use suitable utensils such as deli tissue, spatulas, tongs, single-use gloves, or dispensing equipment except for if the ready-to-eat food is to be cooked in the food service establishment to heat all parts of the food to a time/temperature of at least 145°F (63°C) or if it contains a raw animal food it must be heated to the minimum cook temperature of the raw animal food.
Corrective Actions: The foods were discarded, and the the employee was informed. //
2-2A: .03(1)(c)2,3 and .03(1)(c)17 The person-in-charge was not aware of the employee health policy. // Person in charge shall explain their responsibility for preventing the transmission of foodborne disease by a food employee who has a disease or medical condition that may cause foodborne disease and can describe the symptoms associated with the diseases that are transmissble through food. They also shall be able to explain how food employees and conditional employees comply with reporting responsibilities and exclusion or restriction of food employees.
Corrective Actions: The employee health policy was reviewed with the person-in-charge. //
2-2D: .06(2)(o) Observed the handwashing sinks being used for: 1) obtaining water; and) discarding used water to clean the frost machine. // A handwashing facility may not be used for purposes other than handwashing.
Corrective Actions: The person-in-charge was informed. //
2-2D: .07(3)(b) The men's restroom did not have paper towels supplied. // Each handwashing sink shall be provided with individual, disposable towels, a continuous towel system that supplies the user with a clean towel, or a hand drying device.
Corrective Actions: Paper towels were supplied. //
4-2A: .04(4)(c)1(iv) Several containers of ready-to-eat foods were stored in the walk-in cooler without any food covering. // Food must be protected from cross contamination by storing the food in packages, covered containers, or wrappings, except for loosely covered, or uncovered containers in which food is being cooled if protected from overhead contamination.
Corrective Actions: The foods were covered. //
6-1A: .04(6)(f) Vanilla frosty base and raw grill chicken being kept cold for safety were observed with temperatures above 41°F. // Time/temperature control for safety food shall be maintained at 41°F (5°C) or below.
Corrective Actions: The food was discarded. //
6-1C: .04(6)(d) Diced tomatoes and chopped sandwich lettuce were cooling for more than 4 hours. The foods had temperatures above 41°F. // Time/temperature control for safety foods prepared from room temperatures or refrigeration temperatures shall be cooled from 70°F to 41°F (5°C) or below within 4 hours.
Corrective Actions: The foods were discarded. //
8-2B: .07(6)(c) Observed a packet of chemical stored on shelving above grill (near HennyPenny). // Poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, and single-service and single-use articles.
Corrective Actions: The chemical was removed. //
8-2B: .07(6)(g) Quat sanitizer in wet wiping cloth bucket was too concentrated (over 400 ppm). // Chemical sanitizers used on food contact surfaces shall not exceed stated concentrations (100 ppm for chlorine and 400 ppm for quaternary ammonia).
Corrective Actions: The solution was remade. //
9-2: .10(5)(c) 1. The sliced american cheese was being held under the variance. However, the variance letter is not on-site. 2. The sliced cheese was not labeled with a start or discard time. // "If granted a variance or a HACCP plan is otherwise required, the permit holder shall: 1) Comply with the HACCP plans and procedures that are submitted and deemed in conformance with DPH Rule 511-6-1-.02(6)(a) through (e) as a basis for the modification or waiver; and 2) Maintain and provide to the Department, upon request, records specified under DPH Rule 511-6-1-.02(6)."
Corrective Actions: The food was discarded. Please obtain the letter. //
12A: .04(4)(q) Observed boxes of food stored in direct contact of the floor in the walk-in freezer. // "Food shall be protected from contamination by storing the food: 1) in a clean dry location; 2) where it is not exposed to splash, dust, or other contamination; and 3) at least 6 inches (15 cm) above the floor."
Corrective Actions: The food was elevated off of the floor. //
12B: .03(5)(g) Observed employee preparing food while wearing bracelets. // Food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food (except for a plain ring such as a wedding band).
Corrective Actions: The employees removed the bracelets. //
12C: .04(4)(m) Observed wet wiping cloth container with a film of oil on the surface and cloudy. // Dry wiping cloths and the chemical sanitizing solutions in which wet wiping cloths are held between uses shall be free of food debris and visible soil.
Corrective Actions: The solution was changed. //
13A: .02(1)(d) The inspection report from November 5, 2019 (73C) was not on display in either the drive-thru or the dining area. // At food service establishments with no primary or public door, the current inspection report shall be prominently displayed at all times where the documents can be read by the public from a distance of one foot away. If requirements of this paragraph are not possible because of physical restrictions, a location will be determined as approved by the Health Authority. Food service establishments with drive-thru windows will post the current inspection report, and also have the inspection report posted so that a minimum of the top one-third of a copy of the current inspection report is visible through each window allowing customers to easily read the score, date of inspection and establishment information.
Corrective Actions: Please ensure this report is put on display in both areas. //
15A: .05(6)(a) 1. The upright freezer door does not close properly. Gap observed between gasket of the door and the unit. 2. The frosty machine observed holding the vanilla frosty in the hopper at temperatures above 41°F. // Equipment must be in good repair and properly adjusted.
Corrective Actions: Ensure all equipment is in good repair. //
15C: .05(7)(a)2,3 Observed oily residue on the exterior non-food contact surfaces of sheet pans. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
Corrective Actions: The pans were cleaned during the inspection. //
17B: .06(5)(o),(p) Refuse was observed littered around the dumpster. // A storage area and enclosure for refuse, recyclables, or returnables shall be maintained free of unnecessary items and clean.
Corrective Actions: Please clean the enclosure. //
17B: .06(5)(n) The dumpster doors and one of the top lids were left open. Trash in unit; partially empty on one side. // Outside receptacles and waste handling units for refuse, recyclables, and returnables must be kept covered with tight fitting lids or the doors.
Corrective Actions: Ensure the dumpster doors and lids are closed.
17D: .07(4)(b) Opened employee single-use bottles of water were stored in the reach-in cooler and walk-in cooler over food. // Areas designated for employees to eat, drink, and use tobacco shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination.
Corrective Actions: The beverages were discarded. //
18: .07(2)(m) Observed the drive thru window being left open on multiple occasions while not actively in-use. // "Outer openings of a food service establishment shall be protected against the entry of insects and rodents by: 1) filling or closing holes and other gaps along floors, walls, and ceilings; 2) closed, tight-fitting windows; and 3) solid self-closing, tight-fitting doors."
Corrective Actions: The window was closed each time it was addressed. The district manager stated that the doors have been scheduled to repaired to be automatically closed. //

Employees must drink from disposable cups with secure lids and straws and stored in a manner that prevents contamination of equipment, food, and single-use items.
2-2D: .06(2)(o) Plates stored in handsink. Plates removed. Handsinks may only be used for washing hands.
4-1A: .04(4)(c) unwashed vegetables stored in the same container with washed and cut vegetable in prep cooler. In the walk-in cooler unwashed vegetables stored above ready-to-eat on all shelving in walk-in cooler. Unwashed vegetable must be stored below ready-to-eat.
4-1A: .04(4)(n)1 Empployee cracked shelled eggs and with same gloves touhcing clean equipment and food containers// employee put on hat and removed cup then contiune to make sushi. Employee removed gloves and washed hands sushi discarded. Single-use gloves may only be used for one task and discarded when they become contaminated..
4-2B: .05(7)(a)1 Ice mahcine has mold-like accumulations. Food contact surfaces must be clean to sight and touch.
6-2: .04(6)(g) Several TCS foods located in cooler(soups, porrige, pepper sauces, ect..) not date-marked per owner foods were cooked at 8am yesterday. Foods held more than 24hrs must be date-marked.
8-2B: .07(6)(c) WD-40 container stored above open box of potatoes// WD-40 stored over clean plates// butane lighter stored over prep tables and food. Chemicals relocated. Chemicals must be stored below food and equipment.
10D: .04(4)(d) Several food items removed from bulk supply not labeled(salt, sugar, spices, oil, soy sauce, oyster sauce etc..) located in mulitple prep areas in facility. Food removed from bulk supply must be labeled with common English name.
12A: .04(4)(h) In waitress station bowl of lemons stored in ice use for drinks. Ice used as an exterior coolant may not be used as food.
12A: .04(4)(q) Food delivered yesterday stored on the floor in facility and walk-in freezer. Food must be stored 6" above the floor.
12A: .04(4)(q) Several containers of food and sauce bottles stored in water in prep cooler. Iitems relocated. Food must be stored in a clean dry location.
14B: .05(10)(e) 1,2,4 Cups stored with lip contact surfaces in contact with the dump grid of drink machine. Cups relocated to dish machine. Cups must be stored on clean surfaces.
