Observations: Ready-to-eat food from a commercially processed, hermetically sealed container not heated to a temperature of at least 135oF for hot holding. Canadian bacon reheated to 100F on the grill & then placed into the hot holding unit. Training provided. Canadian bacon placed back onto the grill to continue reheating. Grease/buildup on the cookline equipment. Flies observed in the kitchen, front service counter, dish area, dining room: house flies, fruit flies.

Observations: Mold observed on strawberries stored in the line cooler. French fries adulterated when employee handled raw hamburgers & immediately handled the French fries with his gloved hands. Mold-like buildup inside the ice machine; behind the front panel. Single use cups, which were used as scoops, were left in contact with food in the bulk food containers. Single-use gloves used for multiple tasks that present risk for cross-contamination. Using gloved hands, employee placed raw hamburgers on the grill, handled seasoning container & then handled French fries; he did not wash his hands nor change his gloves.
