OBSERVED NO PAPER TOWELS OR OTHER HAND DRYING PROVISION AT HAND SINK AT START OF INSPECTION. OBSERVED MULTIPLE DENTED CANS IN FOOD STORAGE SEVERAL DENTED ON SEAMS. CANS REMOVED TO AREA NOT IN FOOD STORAGE. OBSERVED POTENTIALLY HAZARDOUS COOKED READY TO EAT FOODS STORED BELOW RAW ANIMAL FOODS IN WALK IN COOLER. OBSERVED RAW ANIMAL FOOD NOT SEPARATED FROM EACH OTHER DURING STORAGE IN WALK IN COOLER. WALK IN COOLER STORAGE WAS RE ARRANGED TO CORRECTLY STORE FOOD. OBSERVED MULTIPLE CONTAINERS AND BAGS OF UNCOVERED FOODS IN WALK IN COOLER AND FREEZERS. OBSERVED METAL BOWL BEING USED TO PORTION POTENTIALLY HAZARDOUS FOOD FOR WOK RINSED IN 3 COMPARTMENT SINK ONLY. WASH, RINSE, AND SANITIZE PROCESS NOT USED. OBSERVED NOODLES IN WALK IN COOLER AT 56F AFTER COOLING OVER 6 HOURS. NOODLES DISCARDED. COOKED POTENTIALLY HAZARDOUS FOODS SHALL BE COOLED FROM 135F TO 41F WITHIN A TOTAL OF 6 HOURS. OBSERVED MULTIPLE COLD HELD READY TO EAT POTENTIALLY HAZARDOUS FOODS HELD IN WALK IN COOLER WITHOUT DATE MARKING. OBSERVED BOTTLE OF DISH SOAP STORED ON MEAT PREP SINK. OBSERVED SHRIMP THAWING IN STANDING WATER IN 3 COMPARTMENT SINK. WATER TEMPERATURE WAS 43OF. SHRIMP REMOVED AND PLACED IN WALK IN COOLER TO COMPLETE THAWING. OBSERVED NO THERMOMETER IN ESTABLISHMENT AND NON PROVIDED WHEN ASK AND TOLD TO CHECK FOOD TEMPERATURE DURING COOLING PROCESS. OBSERVED EMPLOYEE COOKING FOOD AND PACKING RICE IN TO GO CONTAINERS WITHOUT PROPER HAIR RESTRAINT. OBSERVED MULTIPLE WET WIPING CLOTHS ON FOOD PREP TABLE. OBSERVED INSPECTION REPORT DISPLAYED BEHIND PLANT AT REGISTER. ESTABLISHMENT IS NON SMOKING. NON SMOKING SIGN IS NOT POSTED AT DOOR. OBSERVED WALLS AND FLOOR WITH DEBRIS BUILD UP. OBSERVED VENT HOOD FILTER, FRONT OF VENT HOOD, AND SPRINKLER PIPE AT FRONT OF VENT HOOD WITH EXCESSIVE GREASE BUILD UP.
