Violations: 6-1B, Violation of Code: [.04(6)(f) ] Foods not maintained at 135F or higher on steam table: Turnip greens and dressing. Discard the dressing and reheat the turnip greens. No time control or documentation in place for holding those foods. Foods held hot for safety must be maintained at 135F or higher.  Corrected On-Site.  New Violation. 6-2, Violation of Code: [.04(6)(h) ] Prepared foods are being held in use past their refrigerated shelf life: pizza sauce, dressing, chicken salad, cooked pasta, and special pizza sauce from an event. Discard these foods. Ready-to-eat TCS foods must be held only 7 days in refrigeration (the date of preparation is Day One of the 7 days shelf life). Day One for commercially packaged TCS foods is the date on which the package was opened.  Corrected On-Site.  New Violation. 10D, Violation of Code: [.04(8)(b) ] Several foods held in use past the manufacturer's expiration date: mustard potato salad (5/27/2013), egg salad (5/21/2013), and case of cream cheese packets (5/21/2013). Discard the food items. Ensure first-in, first-out method to ensure no expired foods are being used in the restaurant.  New Violation. 15A, Violation of Code: [.05(6)(q) ] Residential refrigerator is not maintaining proper temperature for holding TCS foods. Remove them (milk, cream cheese, etc). Repair the refrigerator or use it to store only non-TCS foods. Fan in lower compartment was not blowing adequately. Freezer compartment was functional.  New Violation. 15C, Violation of Code: [.05(7)(a)2,3 ] The interior of the prep cooler and underneath the lid of the prep cooler is unclean with food pieces. Clean the equipment surfaces. Equipment must be kept free of accumulations of soil, debris, or mold.  Repeat Violation. 17C, Violation of Code: [.07(5)(a),(b) ] Filters of ventilation hood need to be cleaned. Flooring throughout the prep area and behind dry storage items needs to be cleaned. The maintainence of the physical facility must be frequent enough to prevent build-up of dirt, residue, and food debris.  New Violation.

Violations: 4-1A, Violation of Code: [.04(4)(c)1(i)-(iii),(v),(viii) ] (1)An employee was observed placing a raw chicken wing tong in the water (148F) with cooked chicken wing tong. (2) Raw shell eggs were observed stored over blue cheese and "secret sauce". Raw animal foods must be separated from each other during storage, preparation, holding, and display. The employee decided to remove the water bath and eggs.  Corrected On-Site.  New Violation. 4-2B, Violation of Code: [.05(6)(n) ] In-use dish machine temped 135F in main prep area (the booster heater was turned off). Hot water sanitization must meet minimum requirement of 160F. The booster was turned on to reach 162.5F and the plates were removed from the shelf for sanitization.  Corrected On-Site.  New Violation. 6-1A, Violation of Code: [.04(6)(f) ] Onion batter, chicken batter, raw chicken wings, etc. (see temperature observation section) holding since 11:00 AM temp 44F-56F 2:02 PM-3:30 PM in ice baths or prep coolers located on the cook line.  Corrected On-Site.  Repeat Violation. 6-1D, Violation of Code: [.04(6)(i) ] In-house garlic-in-oil wing sauce and other wing sauces holding at room temperature without time control stamps in place or written procedures for time as a public health control. Written procedures for use of time only as a control for potentially hazardous foods must be prepared in advance and maintained in the establishment. Also, the foods must be marked to indicate discard times. The employee decided to discard the foods.  Corrected On-Site.  New Violation. 6-2, Violation of Code: [.04(6)(g) ] Containers of potato soup, macaroni and cheese, etc. with preparation dates of 5/27 and discard dates of 6/4 in the walk-in cooler. Refrigerated, ready-to-eat, potentially hazardous foods must be discarded within 7 days from the date of preparation. The employee decided to correctly date mark the foods.  Corrected On-Site.  New Violation. 6-2, Violation of Code: [.04(6)(h) ] Honey butter prepared on 5/4 and sausage gravy prepared on 5/12 in the reach-in cooler and walk-in cooler. Correctly date-marked refrigerated, ready-to-eat, potentially hazardous foods must be discarded within 7 days from the date of preparation. The employee decided to discard the foods.  Corrected On-Site.  New Violation. 12B, Violation of Code: [.03(5)(i) ] (1) An employee was observed preparing chicken wings with about 2 inch long hair. (2)Employees were observed slicing celery with about 3 inch long hair. Employee must use an effective means of restraint for hair, beard, or mustache longer than ½ inch. The employee donned a hair net for preparing wings.  New Violation. 12B, Violation of Code: [.03(5)(g) ] An employee was observed dressing entrees with guacomole and pico de gallo while wearing a black watch. Employee must wear a plain ring only while preparing food. The employee decided to remove watch.  Corrected On-Site.  New Violation. 15B, Violation of Code: [.05(6)(d),(e) ] The dish machine in the bar area temped 94F. The warewashing equipment must work properly.  New Violation. Correct By: 06/07/2013
