Second Pfizer Dose Clinic To Be Held Saturday In 19th Ward
Those who got their first dose last month in the 19th Ward are automatically scheduled for their second shot May 15 at Mt. Greenwood Park.
CHICAGO — The 19th Ward is hosting a second dose COVID-19 vaccine Clinic May 15 at Mount Greenwood Park, 3721 W. 111th St.
If you received the first dose of the Pfizer vaccine at the April 24 clinic, you are already scheduled for your second dose at the same time as your first appointment. Bring your CDC card and completed vaccine authorization form with you this Saturday.
Those who received their first dose of the Pfizer vaccine elsewhere but would like to get your second dose at Saturday’s clinic, email your name, address, phone number and date of birth to mattoshea@the19thward.com for an appointment. This clinic is for second doses only. There will be no first doses of vaccine given.
