New Health Center Opens In Hermosa
Oak Street Health will expand access to holistic healthcare for Medicare patients on the West Side, the center said.

CHICAGO — A new health center opened its doors Thursday in the Hermosa neighborhood. The center is Oak Street Health's 17th location in the Chicago area. The facility, at 4417 W Diversey, will offer Medicare patients a "more holistic healthcare option," the center said.
In addition to primary care, such as podiatry and behavioral health, patients will have access to services such as complimentary transportation to and from appointments, and Medicare education classes to help them understand their insurance benefits.
To combat isolation and depression among seniors, the center will provide social activities and classes for patients. It will also offer a 24/7 patient support line, care teams and home nursing visits for patients.

“Expanding into the Hermosa community is an exciting step for Oak Street Health! We’re looking forward to offering local community members a new healthcare option that’s personalized to their unique needs,” said Ana Cesan, Regional Vice President at Oak Street Health, in a statement. “We’re also looking forward to partnering with community organizations like the Northwest Side Housing Center to make an impact that goes beyond delivering exceptional care.”
Oak Street Health said it's reduced its patients' hospital admissions by 44 percent, and reduced emergency room visits for its patients by 46 percent.

To provide a more local, community experience, more than 80 percent of the Hermosa center's physicians and staff live within five miles of the center, Oak Street Health said.
