Eight Lake View Restaurants Fail Food Inspections
Patch combed through food inspection reports of local restaurants, cafes and grocery stores. The results are available here.

Patch examined some of this October's food inspection reports from Lake View, and found that eight restaurants in the neighborhood have failed inspections since Oct. 1.
The information has been collected from publicly available data published by the Chicago Department of Public Health's Food Protection Program. The data is available here.
To fail an inspection, the establishment had to have serious violations that weren't correctable during the inspection. A failed rating does not mean that the restaurant's license is automatically suspended.

Rise Restaurant, 3401 N. Southport Ave.
Inspection Date: Oct. 17Results: Fail
Comments: Observed black slimy substance in ice machine; door not rodent-proof; frozen meat stored in plastic grocery bags (should be stored in food containers); oven, wire racks in the cooler and grease container must be cleaned. (Other offenses can be read here.)
Fennel and Iris, 1652 W. Belmont Ave.
Inspection Date: Oct. 17Result: Fail
Comments: Door leading to dumpster not rodent-proof; entry to the food-prep area not protected from the public; food green poison blocks are on the floor in a room connected to the business. More comments are available here.

Comments: (Original) Black slimy substance on wall of ice machine; ice not protected within machine; approximately 30 live fruit-flies were spotted near the sushi-prep area, the main dining room, the kitchen and the bar; walk-in cooler doesn't have proper sushi tags; dish machine is not in use at the time but it was tested and found that it wasn't dispensing sanitation properly; brighter lights needed by ice machine and walk-in coolers.
(re-inspection) Outer openings not insect/rodent-proof. It appears that other violations had been repaired during the re-inspection.

Comments: Fan covers need to be cleaned; water leak in the kitchen area; unnecessary articles stored in the cooler; light shields must be cleaned; back door is not rodent-proof
Waterhouse Tavern and Grill, 3407 N. Paulina

Comments: Approximately 20 live fruit flies were seen on the equipment, walls and ceiling of the bar and dining room, pest control service is needed.
Old Crow, 3506-3512 N. Clark

Comments: Outer openings need to be protected from rodents/insects; around 25 live fruit flies were seen; prep table, walk-in cooler, dishmachine, bar all require cleaning. Note: Roudhouse 66 did pass their follow-up inspection.

Comments: Hood filters above cooking equipment needs a deep cleaning; basement needs to be cleaned; outer openings need to be protected from insects/rodents; much of the business needs a detailed cleaning
Editor's Note: A previous edition of this story incorrectly stated that Roadhouse 66 had to install a grease trap and hand sink in the restaurant. This is incorrect, as the restaurant has both of those things. This article has been updated to reflect that.
