To fail an inspection, the establishment had to have serious violations that weren't correctable during the inspection. A failed rating does not mean that the restaurant's license is automatically suspended.

Comments: No comments were available for the Oct. 30 inspection. For the Nov. 5 inspection, the comments note that there was no handwash sink in the rear dishwashing area and the food prep area must be protected from customer interaction. Doors must cover gaps to prevent rodents and insects from entering. The outside garbage dumpster was overflowing, preventing the lid from closing properly. 

Fairways, 1139-1141 W. Armitage Ave.

Comments: Walk-in cooler must be replaired, and must maintain an air temperature of 40 degrees or below (critical violation). There's no evidence of the outer openings being protected from rodents/insects; the garbage dumpster was overflowing (no citation issued).
