These registration links for vaccine clinics are for CHELMSFORD RESIDENTS AGE 75+ ONLY.

Choose a clinic below and make sure you register for BOTH a 1st and 2nd dose (separate links for each). If you need assistance with on-line registration, please reach out to family and friends. DO NOT CALL OR COME IN TO THE BOARD OF HEALTH TO REGISTER.

3. For 2nd Dose Bring Vaccination Card (vaccination card given with 1st dose)
