Join this important community forum to talk about what health needs and concerns you have regarding your community.

GRAFTON, MA—What do you think Grafton needs in terms of healthcare?

Fallon Health, UMass Memorial Medical Center and the Central Massachusetts Regional Public Health Alliance have partnered to conduct the 2018 Greater Worcester Community Health Assessment (CHA). A CHA is conducted every three years to help us understand key problems that affect our health and also assesses the strengths of our community. The last CHA was completed in 2015 and it has played a vital role in addressing health needs in our region.

Now, there are three forums scheduled to update this assessment.
