Coronavirus Vaccine Clinic Now Open In Hopkinton In Town Of Northborough

The Hopkinton Health Department is hosting COVID-19 Vaccine Clinics on the following dates and times. These clinics are open to all Massachusetts residents aged 75 and older (in addition to Phase 1 individuals). Please click on your preferred date and time to book an appointment. The clinic will be at the Hopkinton Senior Center, located at 28 Mayhew Street. You must have an appointment prior to arriving at the clinic.

Tuesday, February 16, 2021 from 12:00PM to 4:00PM
These clinics will be offering Pfizer first dose vaccines only. Please read the attached EUA fact sheet and Pre-Vaccination form and contact your primary care physician if you have any questions. Potential side effects and contraindications are listed on the pre-vaccination forms. The Pfizer vaccine requires two doses that are given approximately 21days after the first dose. You will be asked to schedule your second dose at checkout, following your initial vaccination.
We will take all necessary precautions to safely deliver the COVID-19 vaccine, including wearing personal protective equipment and frequently sanitizing surfaces to ensure the safety of our residents and staff. Please do not come to the clinic if you are sick, there will be future clinics.
If you are 75+, you should:
· Bring a photo ID that includes your name and date of birth
· Bring your birth certificate
· Your insurance card
If you do not have any of these forms then please print the state self-attestation form to verify your eligibility for the vaccine.
