Sudbury wants to fix Americans with Disabilities Act (ADA) problems at town facilities.

In the next 3 to 5 years Town leaders must address accessibility problems identified in the recent inventory of all town facilities and services. The information you contribute will help clarify what’s most urgent. You will help set the priorities.

Be part of the Sudbury ADA Self Evaluation and Transition Plan project either by emailing your own comments or by filling in one of the surveys in a PDF file listed below and sending it as an attachment to

ada@sudbury.ma.us

To submit a survey:

  1. Choose the applicable survey from the three below.
  2. Fill in the PDF form.
  3. Save/download to your device in a location you can then find.
  4. Create an email to ada@sudbury.ma.us
  5. Attach the survey PDF file.
  6. Send by Wed, April 14.
