Shrine Catholic Athletes Sports Physicals Aug. 3

Students in grades 5-12 who will be playing sports for Shrine Catholic Grade School or Shrine Catholic High School & Academy in the fall or winter (this would include soccer, football, basketball, volleyball or cheerleading) must have a physical on file before beginning play.

The Dads’ Club will have a pediatrician who will be doing sports physicals 3-9 p.m. Aug. 3 at Shrine Catholic High School’s Gymnasium. Athletes should enter the main doors of the gym (signs will be posted).

There will be a $20 fee per athlete (all money will go into the Dads’ Club fund, so checks should be made out to “Shrine Dads' Club”).

Students should wear a T-shirt and shorts. They should bring their green physical form (if they are Shrine Catholic Grade School or Shrine Catholic Academy students) they received at the sports “call out” in May or the high school sports physical form (if they are Shrine Catholic High School students) they received in the packet which was mailed to them. If they do not have the form, forms will be available. Parents should fill out the front side of the athlete’s form (please include the immunization record).

Sign-up sheets with time slots are available on a first-come, first-served basis.
