MN Vaccine Incentives: Request Form Opens Tuesday

"Requests are open and we're ready to give out some rewards," Gov. Tim Walz said Tuesday.

MINNESOTA — The rewards request form for Minnesota's "Your Shot to Summer" vaccine incentive campaign opened Tuesday.

Minnesota residents who are 12 and older — and who have received their vaccine dose since May 27 — can fill out the form and choose their "vaccine reward."

"Requests are open and we’re ready to give out some rewards," Gov. Tim Walz said in a news release Tuesday.

"We know everyone’s excited to get to their summer plans — and if you get vaccinated now, we can help add another event to your calendar for free. Getting your shot now is easier than ever and the key to keeping our state safe for everyone. Help yourself, help our state, and take your shot to summer!"

Rewards will be available for the first 100,000 requests made by eligible Minnesotans. Reward requests must be submitted to the Minnesota Department of Health by 11:59 p.m. on June 30, 2021, and will be fulfilled on a rolling basis.

"If you still need to get your COVID-19 vaccine, now is the time to get double rewards! You will not only get protected for yourself and your community – you will also get a free pass to some of our most favorite Minnesota pastimes," said Lt. Gov. Peggy Flanagan.

"We’re excited to get these rewards out and build more community protection as Minnesotans take their shot to summer."

In addition to the "Your Shot to Summer" program, if you received the COVID-19 vaccine, you now qualify for a free alcoholic beverage in Minnesota through the "Cheers to the Vaccine" campaign.
