Cutting board heavily scratched and soiled. Scratched cutting boards shall be resurfaced or discarded. Correct by June 21. Three-compartment sink's leaking around nozzle. Plumbing systems shall be in good repair. Correct by June 21.

Using time as a public health control but timer said 8 hours with no time log for verify. 4 hours is maximum time foods can be served out of temperature. Discarded rice and started with fresh batch. Time log for sushi rice not being used. HACCP plan for sushi rice shall be available or logs for sushi rice serving times shall be available and completed in order to have rice out of proper temperatures for longer than the 4 hour window.

Put log out for employees to start using for time verification. Several food containers in sushi prep area in front of house not labeled. All food stored out of original container shall be labeled with common name. Correct by end of business today. Handsink in kitchen has broken nozzle. Hand sink valves, in front area, is also leaking. Plumbing systems shall be in good repair. Correct by June 27
