A thorough initial fertility workup

Your first appointment with your reproductive endocrinologist will consist of your doctor learning as much as possible about your medical and family history, and performing a physical exam to check the health of your reproductive system. This includes a pelvic exam and a vaginal ultrasound to see your uterus and ovaries. You’ll also learn about testing you may need to do at a follow-up appointment, such as blood work to determine your egg reserve, a hysterosalpingogram (HSG) to evaluate whether your fallopian tubes are open, and, if you have a male partner, he’ll be asked to give a semen sample for analysis.

A diagnosis you understand

Once all your testing is done, your fertility doctor will present you with a diagnosis and a recommended plan of action, and the two of you will decide on a way forward together. A few common causes of infertility include: Diminished Ovarian Reserve (DOR), where a woman has a low egg supply; Polycystic Ovary Syndrome (PCOS), where a woman has trouble growing and releasing a mature egg; fibroids or polyps, which are growths that can prevent embryo implantations in the uterus; endometriosis, a condition where cells from the endometrial lining grow outside of the uterus and cause scarring and inflammation that can prevent the meeting of sperm and egg or the implantation of an embryo; and a low motile sperm count, which can prevent fertilization.


A fertility treatment plan right for you

While there are many causes of infertility, treatment is available, and patients should have hope no matter their diagnosis. There are four treatment paths for patients. The first is natural cycle monitoring, or when a doctor suggests the best time to have sex based on the woman’s fertility. The second is when a doctor prescribes medication that will help a woman ovulate, and suggests when to have sex. Next are the two most common procedures: Intrauterine Insemination (IUI) and In Vitro Fertilization (IVF). During IUI, a woman takes medication to ovulate, and doctors insert sperm into her uterus at just the right time. In the case of IVF, a woman’s egg is removed, fertilized with sperm in a lab, and one of the resulting embryos is placed back into her uterus at the most receptive time.
