TEANECK, NJ - The Township of Teaneck and Holy Name Medical Center are committed to increasing access to the COVID-19 Vaccine to eligible individuals with special health care needs including those with physical and intellectual and developmental disabilities who may need special accommodations beginning on Monday, April 5, 2021.

To register; patients must be 18 years of age or older, have conditions and special needs that would prevent them from obtaining the vaccination from a public facility. The Johnson & Johnson, single dose vaccine will be administered. Documentation from a medical provider, social worker or home care program administrator is required.
