Mamaroneck Requires Coronavirus Daily Health Screening

MAMARONECK, NY — To prevent the spread of the new coronavirus in district schools, everyone is required to complete a COVID-19 screening. The form must be filled out and submitted everyday prior entering any district facility.

The COVID-19 Health Screening Attestation will be required in addition to a temperature check upon arrival at school.

There are other ways to get to the daily health screen form.

1. Via the Mamaroneck School District app homepage (select "Health Form" - 4th tab on bottom). Download the app by searching "Mamaroneck Public Schools" on the App Store, then selecting "Get." Hommocks and Mamaroneck High School students may be asked upon arrival to verify that they have completed and submitted the daily health screening form; by having this information on their phone, they can easily access it upon request.
2. Access the COVID-19 Health Screening Attestation form link through the QR link at the bottom of the form on your cellphone and add it to your phone's homescreen by following these three steps: - Tap the square with an arrow pointing up. - Scroll down to "Add to Home Screen" with plus sign in box. - Click "Add" in the upper right-hand corner.
