SUNSET PARK, BROOKLYN — A seafood restaurant on Eighth Avenue has been shut down by health inspectors for the third time this year, records show.

Happy House Seafood Restaurant received 45 violation points during an inspection late last week, nearly twice the 28-point threshold that requires the Department of Health and Mental Hygiene close a business.

The failing grade comes just two weeks after inspectors gave the eatery 38 violation points on Oct. 31 and about six months after it was given a whopping 81 violation points during an inspection in May, records show.

In all three inspections, vermin seemed to be the problem.

Last week's inspectors found that there was evidence of mice or live mice in the eatery, live roaches and that the facility was not vermin proof.

Mice were also a violation point back in October, while mice, roaches and filth flies were all on the list of violations during the May inspection, records show. The inspectors said the restaurant was not vermin proof all three times, records show.

The inspectors last week also cited the eatery for having inadequate personal cleanliness, a food contact surface that was not properly washed and a non-food contact surface that wasn't properly constructed, records show. The food-contact surface not being washed was a violation on all three inspections.

The restaurant did not immediately answer a request for comment about the inspections.
