Oregon DOC Inmate Dies; 4th Death In One Month

Two inmates at Coffee Creek Correctional Facility in Wilsonville have died in custody since Sept. 26. Two others died at other facilities.

WILSONVILLE, OR — For the fourth time in as many weeks the Oregon Department of Corrections on Wednesday reported the death of another inmate, this time at the Coffee Creek Correctional Facility in Wilsonville.

Sally Ann Bicandi, 54, reportedly died Tuesday night at hospital. Her cause of death has not yet been released.

Bicandi is the second inmate at Coffee Creek to die while in custody recently. On Sept. 26 Oregon DOC officials reported the death of 27-year-old Adam Montgomery Farmer, who on Aug. 24 was sentenced to two years in prison for unlawful use of a weapon, a Class C felony. On Sept. 13 Farmer was given an additional 13-month sentence for identity theft he committed in March 2018.

Farmer was scheduled for release on March 24, 2021.

Bicandi was in the middle of serving a three-year sentence for two counts of attempted assault and criminal mischief. Her earliest release date was November 7, 2019.

Elsewhere in the state, the DOC reported the deaths of Douglass David Miller, 72, and Raymond Madrigal, 81. In 2006 both men were sentenced in a Multnomah County court to more than eight years for various, unrelated sex crimes.

Miller was scheduled for release from the Oregon State Penitentiary in Salem on Jan. 11, 2021. He reportedly died in the prison's infirmary.

Madrigal was scheduled for release from Two Rivers Correctional Institution in Umatilla on Aug. 8, 2022. He reportedly died in hospital.
