Tri-County Health Center Scheduling Vaccines: Link To Form Here

Tri-County Health and Wellness Center in Phoenixville ask residents of Chesco, Montco, and Delco to sign up for a COVID-19 vaccine.

PHOENIXVILLE, PA — Tri-County Health and Wellness Center has announced it is administering COVID-19 vaccines by appointment.

Tri-County Health and Wellness Center, 507 Kimberton Road, said it offers the Moderna COVID-19 vaccine to Pennsylvania residents in Phase 1A, who are 18 and older, and live in Chester, Montgomery, and Delaware counties.

Those in Phase 1A as described who want to be added to Tri-County's waiting list to receive the vaccine, need to complete one pre-registration form for each person requesting the vaccine.

After submitting the form, residents will be contacted and can schedule an appointment, according to a statement from Tri-County Health and Wellness. The center is scheduling up to 40 patients per day.

"We ask for your patience since the demand for vaccines is very high," they said.
