Important information This is a Reminder - If you are scheduled for a SECOND DOSE of Pfizer Vaccine on WEDNESDAY, 6/16 at the GIANT FOOD STORE -  Location:  UPSTAIRS COMMUNITY CENTER ROOMS located at 315 YORK ROAD, WILLOW GROVE, PA.  Appointment times for Dose 2 on WEDNESDAY 6/16 are the same as the appointment times from Dose 1 on given on WEDNESDAY, 5/26.  To be fully protected against the Covid-19 virus, it is very important that you receive your second immunization within 42 days of your first immunization NOTE: You will need to complete ANOTHER Informed Consent form for this appointment. Please also bring your white CDC Covid Vaccine Vaccination record card with you to this appointment.  Any patient under the age of 18 MUST have a parent or guardian present.
