Critical violation - Observation (CORRECTED DURING INSPECTION): The following equipment/utensils were observed soiled to sight and touch: Observed a container on shelf of cleaned food contact surfaces with measuring cups, tongs, dispensing bottle with food debris on them
Critical violation - Observation (REPEAT): Observed the lack of an approved backflow prevention device on the water supply system. Observed at the mop sink a splitter valve attached to the faucet and a hose attached to the valve running and hanging in a container of water and dirty food equipment
Correction: A plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the food establishment, including on a hose bib if a hose is attached or on a hose bib if a hose is not attached and backflow prevention is required by Law by installing an approved backflow prevention device as specified under 5-202.14. A DOUBLE CHECK VALVE MUST BE INSTALLED ON THIS FAUCET SET UP IF YOU WISH TO USE IT AS IT IS

Non-critical violation - Observation (REPEAT) (CORRECTED DURING INSPECTION): PANS were found stacked while wet ON THE SHELF NEXT TO THE 3 VAT SINK after cleaning and chemical sanitization.
Correction: After cleaning and sanitizing, equipment and utensils shall be air-dried or used after adequate draining before contact with food. DISCUSSED WITH THE STAFF, THE PANS AND OTHER ITEMS ON THE SHELF WERE REMOVED AND STORED INDIVIDUALLY TO AIR DRY.

Critical violation - Observation (CORRECTED DURING INSPECTION): The following food item(s) were found hot holding at improper temperatures using a calibrated food temperature measuring device: chicken 118F in hatco 3 drawer
Critical violation - Observation (CORRECTED DURING INSPECTION): When tested, the chlorine-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the santiizer bucket coffee area with a concentration of 0 ppm total chlorine.
Non-critical violation - Observation (REPEAT) (CORRECTED DURING INSPECTION): The food temperature measuring device was found out of calibration in the range of use.

Critical violation - Observation (CORRECTED DURING INSPECTION): Raw animal food holding in a manner that may cause cross contamination of ready-to-eat food as follows: raw beef observed on top of cooked gyro in walkin cooler
Critical violation - Observation (REPEAT) (CORRECTED DURING INSPECTION): The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: 1) cooked eggplant 48F, hummus 44F, feta 44F, cooked pasta 48F, butter 47F all at true 2dr preptop cooler, 2) butter and sliced tomatoes 45F, heavy cream 47F, cooked pinto beans 45F all at walkin cooler
Non-critical violation - Observation (CORRECTED DURING INSPECTION): Dispensing and/or in-use utensils improperly stored between use as follows: tongs on oven handles
Non-critical violation - Observation (REPEAT) (CORRECTED DURING INSPECTION): The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: True 2dr preptop cooler 47F
Non-critical violation - Observation (CORRECTED DURING INSPECTION): Observed that sign or poster that notifies food employees to wash their hands is not provided at the bar handwashing sink used by food employees.
Non-critical violation - Observation (CORRECTED DURING INSPECTION): Observed that the walls by the grease trap and dish machine are in need of cleaning.
