Puyallup School District: Register Now: Vaccination Clinic For Ages 12 And Up

Puyallup School District is once again partnering with Kirk’s Pharmacy to host a vaccination clinic for youth. The 2-dose Pfizer vaccination is now available for youth ages 12 years and older in Washington state.
The vaccination clinic will be held May 26 at Rogers High School. The second dose will be administered to registrants June 16. To accommodate learning schedules, appointments will be held 7 – 11 a.m. Students receiving vaccines will be excused from homeroom class on vaccination clinic dates.
Signed parental consent forms are required to receive the vaccine. To expedite the sign in process, please bring the linked consent forms below to your appointment time.

Students are required to stay 15 minutes after the vaccine is administered for observation purposes. An appointment timeframe on May 26 will be assigned before the student leaves.
