Nominate a Champion for Women's Health

Wisconsin Women's Health Foundation is currently accepting nominations for their 2012 Champion in Women's Health Awards. The award recognizes exemplary leaders in promoting the quality of life for women.

Nominees may include consumer advocates, volunteers, health care professionals, non-profit employees, or individuals who have overcome a significant health issue and/or have inspired other women to become advocates for their own health. If you know a woman or man who deserves to receive recognition, be sure to complete the nomination process by Thursday, December 1, 2011.

Winners of the award will be honored during a private reception on Saturday, May 5, 2012 and during Wisconsin Women's Health Foundation's 13th Annual Spring Gala immediately following the reception.

The Wisconsin Women’s Health Foundation (WWHF) is currently seeking nominations of women and men who have demonstrated exemplary leadership in improving the quality of life for Wisconsin women and families in the following women’s health areas:
• Cancer • Cardiovascular Disease • Domestic Abuse • Mental Illness • Osteoporosis • Tobacco & Alcohol Cessation • Rural Women’s Health • General Women’s Health

Nominees may include consumer advocates, volunteers, health care professionals, non-profit employees, or individuals who have overcome a significant health issue and/or have inspired other women to become advocates for their own health. Past champions honored have raised the level of awareness about women’s health issues; expanded the base of knowledge about women’s health through community service and professional achievement; and/or impacted the quality of care that Wisconsin women receive.
NOMINATION REQUIREMENTS

  • Complete and return the Nominee Information Cover Sheet.
  • Describe, in 500 words or less, how the individual has demonstrated leadership in improving the quality of life for women and families in a particular area of women’s health.
  • Include a minimum of two, maximum of three supporting documents such as letters of support, news articles or photographs.
  • Past winners are not eligible for nomination; however, nominations will be kept active and on file for two years.
  • Include 8 (eight) copies of the entire application including all supplemental materials (8.5 x 11).
