- Are a Wauwatosa resident
- Can provide documentation of your exposure or your case of COVID-19
If you need a letter from the Wauwatosa Health Department to:

- Identify when it is safe to return to work
- Exclude your child from school
- Identify when it is safe for your child to return to school
Instructions:
- Call 414-479-3543 to request a letter
- Please leave a voicemail with your name, phone number, and date of birth
- The hotline is monitored Monday-Friday, excluding holidays
- A representative from the Health Department will return your call by the end of the next business day
- We will work with you to determine your release date from isolation or quarantine and we will send you a letter with your release date via email
- Please expect up to 3 business days to receive your letter.
