Wauwatosa Health Department Hotline For School Exclusion And Work Release Letters

Many employers and schools are requiring documentation from their employees/students after a positive COVID-19 test or exposure to a positive individual.

The Wauwatosa Health Department can assist you with this documentation if you:

• Are a Wauwatosa resident
• Can provide documentation of your exposure or your case of COVID-19

If you need a letter from the Wauwatosa Health Department to:

• Be released from work
• Identify when it is safe to return to work
• Exclude your child from school
• Identify when it is safe for your child to return to school

Instructions:

• Call 414-479-3543 to request a letter
• Please leave a voicemail with your name, phone number, and date of birth
• The hotline is monitored Monday-Friday, excluding holidays
• A representative from the Health Department will return your call by the end of the next business day
• We will work with you to determine your release date from isolation or quarantine and we will send you a letter with your release date via email
• Please expect up to 3 business days to receive your letter.
