May 12 Update: Due to overwhelming demand, we are adding a drive-thru vaccine clinic for this Sunday at Harper from 10:00 a.m. - 4:00 p.m. Parents and students are encouraged to come on Sunday, May 16 even if already signed up for Tuesday, May 18. The second dose clinic will be on Sunday, June 6 from 10:00 a.m. – 4:00 p.m.

Original Story: A Pfizer COVID-19 Vaccine Clinic for students age 12 and over will be held at Harper Junior High (4000 East Covell Blvd. in Davis) in the gym on Tuesday, May 18, from 2:00 - 6:00 p.m. Please use this Google form to sign your student (12 years and older) up for a vaccine slot. Appointments are not required but completing this form will help inform us on the number of people arriving at once. Any additional unvaccinated family members may also be vaccinated at the same time.

Please be aware that the student must receive the second vaccine shot in addition to this first shot, which will be administered at another clinic held on June 8 from 2:00 - 6:00 p.m. Parents do not need to be present with the student at the vaccine clinic. Any student arriving unaccompanied (without a parent present) must submit the completed and signed COVID-19 Screening Questionnaire.
