Christopher Martinelli, 32, of Henry St., has been arrested by warrant and charged by Trumbull police with driving under the influence, second-degree assault with a motor vehicle and failure to drive right.
Police said Martinelli crossed the yellow line and crashed head-on into another car at Howe Avenue and Park Street in Trumbull on May 17.
The injuries were not life-threatening, police said.
Martinelli's bond was set at $2,500 for an appearance in Derby Superior Court.
