The consent form and information about the vaccine can be found on the Town Of Oxford website at oxford-ct.gov, or you can pick them up at the Oxford Senior Center before Friday at 4:00 p.m. Please bring a copy of your health insurance card and driver's license or photo I.D. to the clinic. If you do not have access to make a copy, the Senior Center will be happy to make a copy for you when you pick up your registration form.

A second dose clinic has been scheduled for March 20th at the same location. Please be sure that you are available for this date to receive your second dose.

Due to social distancing measures, you must arrive only for your scheduled time- do not arrive early. Everyone inoculated will receive a Tootsie Roll Pop. For those of you who are romantics, think of it as a belated Valentine’s Day gift. It is our cheap way of saying WE LOVE YOU.
