• ESTABLISHMENT MUST HAVE SOMEONE OF STAFF CERTIFIED IN FOOD SAFETY AND POSTED IN PLAIN VIEW.
• Observed employee with no hair restraint. All food service workers must wear hair restraint.
• Observed the door handle for the walk-in cooler is broken. Must be repair.
• Observed women bathroom is out of order. Plumbing system is in disrepair, must be repaired as soon as possible.
• Observed nonfood-contact equipment in poor repair. Must patch all holes in the wall to prevent food contamination and rat/roach harborage.
