The manager corrected and discarded sliced tomatoes and mushrooms and relocated cheese dip and sour cream to another cooler.
• Food baskets with grease build up and must be cleaned and sanitized before storage. The manager corrected and relocated basket to the dish sink for cleaning.
• Dumpster improperly left open and must be kept closed.

• Observed 2, 5-gallon buckets of cooked vegetable soup in walk-in at temperatures of 45-49°F after cooling overnight. Contents of both buckets discarded. Potentially hazardous food must be cooled from 135-70°F in 2 hours and from 70-41°F in additional 4 hours.
• Sushi rice in back prep area in warmer at 128°F. Time as control implied for rice, but no times were marked and rice was not correctly listed on time as control document. Rice was correctly marked with discard time (2pm) and added to time as control list.
• Wipe cloth water was heavily soiled with food particles and debris. Water must be maintained in clean state.
• Circular plywood piece used for pressing water from vegetables used for making eggs rolls is in poor repair and is not food grade material. Use only food grade, smooth, non-absorbent, easily cleanable material.
• Bottoms of shelving units and glass door drink cooler are rusted and damaged. Replace/repair equipment that can no longer be maintained.
• Observed non-food contact surfaces with food, grease build-up. Clean and maintain equipment.
• Several damaged floor areas in kitchen. Tile must be repaired.
