COVID-19 Vaccine Clinics Set For April 28 And May 19 In Arlington Heights
Appointments are encouraged for the mass-vaccination event at the Arlington Ridge Center.

The Arlington Heights Park District, the Village of Arlington Heights and the Arlington Heights Memorial Library have partnered with Jewel/Osco and will be hosting a mass vaccination site on Wednesdays, April 28 and May 19, 11 a.m.-5 p.m., at the Arlington Ridge Center (ARC), formerly Olympic Indoor Swim Center, at 660 N. Ridge Avenue.

APPOINTMENTS STRONGLY ENCOURAGED – walk-ins will be accommodated on April 28, 1-3:30 p.m. If interested, please make your appointment immediately as these appointments will fill quickly. All residents of Illinois over 16 years old are eligible for the vaccine. All scheduling of vaccines will be done through this link: https://kordinator.mhealthcoach.net/vcl/1616779375040
Please make an appointment only if you are able to make both dose 1 and dose 2 dates. (You will not need to re-register for the second dose)

Dose 1: Wednesday, April 28
Dose 2: Wednesday, May 19 - same appointment time as first dose, you do NOT need to register for the second dose

From 11 a.m.-5 p.m.
Walk-ins will be accommodated 1-3:30 p.m.

Important Information:
- Pfizer vaccine will be administered.
- You will not be able to enter the building until your scheduled appointment time.
- Have the consent form completed in advance - you will find this in the response email you will receive from Albertson’s (the vaccine distribution company) after you book your appointment. IF you do not see the email from Albertsons then check your Spam Mail or your ALL Mail folders for the email confirmation from Albertsons.
- Individuals that are 16 and 17 years of age will need a parent’s signature on the consent form and have a parent/guardian present to receive a vaccination.
- Wear vaccine appropriate clothing (short sleeves) and face covering to expedite the process.
- For all appointments, bring your Medicare/insurance card and drivers’ license.
