Skokie COVID-19 Vaccine Pre-Registration Survey Launches

The village's registration form follows similar efforts in Lake County, Evanston and suburban Cook County.

SKOKIE, IL — The village's health department launched a COVID-19 vaccine pre-registration survey in order to be notified when they become eligible to make an appointment for a jab.

Village staff invited all Skokie residents over age 18 and any health care workers employed at a facility in Skokie were invited to fill out the form. Proof of residency or employment will be required.

The announcement came shortly after Gov. J.B. Pritzker announced the entire state of Illinois was moving to Phase 1b on Jan. 25. Everyone over 65 and all front-line essential workers are eligible in the second vaccine priority group.

The village's vaccinations will be administered at 7770 Frontage Road, initially by appointment only.

The Lake County Health Department launched its vaccination pre-registration survey on Dec. 9 and has since seen nearly a third of the county's population sign up. Suburban Cook County and Evanston, which each operate their own health departments, launched their vaccine pre-registration surveys last week.

Illinois has nearly 100 health departments, all of which are tasked with developing and executing localized versions of the state's vaccination plan.
