The Hopkinton Health Department is hosting COVID-19 Vaccine Clinics for on the following dates and times. In order to book an appointment, you must be a member of the Massachusetts Department of Public Health’s Phase 1 group or be Phase 2, 75 or older. Please click on your preferred date and time to book an appointment. The clinic will be at the Hopkinton Senior Center, located at 28 Mayhew Street. You must have an appointment prior to arriving at the clinic.

These clinics will be offering Pfizer first dose vaccines only. Please read the attached EUA fact sheet and Pre-Vaccination form and contact your primary care physician if you have any questions. Potential side effects and contraindications are listed on the pre-vaccination forms. The Pfizer vaccine requires two doses that are given approximately 21 days after the first dose. You will be asked to schedule your second dose at checkout, following your initial vaccination.
We will take all necessary precautions to safely deliver the COVID-19 vaccine, including wearing personal protective equipment and frequently sanitizing surfaces to ensure the safety of our residents and staff. Please do not come to the clinic if you are sick, there will be future clinics.
If you are eligible to receive the vaccine in Phase 1, you should:
- Bring a photo ID
- Bring your eligible ID from work or current pay stub
- Your insurance card
If you are 75+, you should:
- Bring a photo ID that includes your name and date of birth
- Bring your birth certificate
- Your insurance card
If you do not have any of these forms then please print the state self-attestation form to verify your eligibility for the vaccine.
