Free Sports Physicals for Salem, Pelham, Windham Athletes

Parents: Salem High School and Essex Orthopaedics/Optima Sports Therapy will once again be sponsoring free sports physicals to all student-athletes entering grades 6 through 12, according to a press release.

This year, the physicals will be held at Essex Orthopaedics on June 2, from 5 to 8 p.m. Essex is located at 16 Pelham Road in Salem, which is in the same building as the WORKOUT CLUB (Please enter at the BACK of the building and do not walk through WORKOUT CLUB).

If you are interested in having your athlete participate in this, please pick up and fill out the form ahead of time. Forms can be found in each school’s main office, or printed from Salem High School’s athletics website sau57.org/shs/Athletics/Documents/AthleticForms.pdf.

This is the only date and time that the orgs will offer the free physicals for 2015/16. If there are any questions, feel free to email Seth Conway, athletic trainer at Salem High School seth.conway@sau57.org.

Completed forms that are signed and received at Optima Sports Therapy by May 31, qualify for pre-registration. Walk-ins are welcome and will be seen on a “first come, first served” basis after pre-registered athletes. Athletes should bathe prior to arrival and dress in shorts and tee shirt.
