Parents/guardians are asked to complete the following forms for each child at the elementary and/or secondary level so that resources may be planned accordingly. The commitment survey should be completed and submitted by July 12. If responses are not submitted by that deadline, it will be assumed students will attend in-person.

If the virtual learning option is selected, the commitment will remain in place for the first trimester (until Nov. 24, 2021 if a child is in elementary or middle school), or for the first semester (until Jan. 21, 2022 if a child is in high school).
