Lost Your COVID-19 Vaccine Card? How To Get A New One In RI
The Rhode Island Department of Health made it easy for residents to view and print a copy of their vaccine record.

PROVIDENCE, RI — Rhode Island's vaccine effort presses on, with more than 559,000 residents fully vaccinated as of Thursday. For those who are fully vaccinated but may have misplaced their card showing their vaccine record, there's no need to panic: there's an easy way to get a replacement.
The Rhode Island Department of Health encouraged Rhode Islanders to take a photo of their vaccination card or make a copy, then store the original in a safe place.
However, if it's too late for that and your card is already missing, there's an easy way to get a new one. Rhode Islanders can access their vaccine record using the state's online vaccine portal, and print a copy for their records. Call 401-222-8022 for assistance.
