Fredericksburg Restaurant Inspections: 25 Violations At One Place

The Rappahannock Area Health District found violations at several locations, including one restaurant in Fredericksburg with 25 violations.

FREDERICKSBURG, VA — Officials from the Rappahannock Area Health District inspected multiple food storage and preparation businesses across the region from June 17 to June 22, including one location with 25 violations.

As coronavirus restrictions continue to loosen around the state, health inspectors are returning to conduct restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department also performs a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

    • Date inspected: June 17
    • Total violations and corrective actions: 1
    • Violation:
      • Observed water flowing out the water piping in the mobile unit when the pump is turned on.
  • Fusion: Caribbean and American Grill, 3925 Jefferson Davis Highway Stafford, VA 22554
    • Date inspected: June 17
    • Total violations and corrective actions: 8
    • Violations:
      • Observed the cook handle raw beef with gloved hands then took the gloves off and attempted to put on a new pair without washing her hands. EHS informed the employee and the person in charge to immediately wash her hands.
      • Observed raw shrimp stored over vegetables in the reach in cooler. The person in charge rearranged the foods.
      • Observed uncovered foods in the reach in freezer and shrimp in the front reach in cooler.
      • Observed the quaternary solution in the 3-compartment sink measured 100 ppm. EHS requested several times for this to be corrected however it was not.
      • Observed ox tails sitting at 72'F on the floor. The person in charge discarded the product.
      • Observed rice and beans, restaurant-made sauces, and beans that were not date marked.
      • Observed ox tails and sauce left out over night to cool. Observed a large stock pot of cooked beans in the rear reach in cooler left to cool. The beans were made on Saturday.
      • Observed a large amount of food in the reach in freezer and the reach in cooler stored directly in to go bags without a barrier.
  • Vinny’s Italian Grill & Pizzeria, 397 Suite #104 Garrisonville Road Stafford, VA 22554
    • Date inspected: June 17
    • Total violations and corrective actions: 3
    • Violations:
      • Observed raw shrimp in a covered container stored over ready to eat sliced sausage on a shelf in the walk in cooler.
      • Observed food contact surfaces of the meat slicer soiled.
      • Observed the following TCS foods holding at improper temperatures in prep cooler #3: Shredded mozzerella (47°F), alfredo sauce (47°F), cooked pasta (50°F, 50°F), cooked lasagna (47°F), cheese manicotti (47°F). The person in charge reported the TCS foods in the unit have been cold holding overnight.
  • Honey Baked Ham & Cafe, 2007 Plank Rd Fredericksburg, VA 22401
    • Date inspected: June 21
    • Total violations and corrective actions: 3
    • Violations:
      • Observed Peracetic Acid vegetable wash solution exceeding manufacturer's concentration. The vegetable wash solution was tested during the inspection. A food employee measured the solution with a testing kit at 9.80 ML, the recommended concentration range on the manufacturer handout and test kit is 5.88 - 7.81 ML
      • Observed open shelving, used to store cleaned and sanitized food contact equipment, stored less than one foot between the warewashing area hand sink and a prep sink.
      • Observed absence of backflow prevention on the water line connected to the ice machine.

    • Date inspected: June 22
    • Total violations and corrective actions: 25
    • Violations:
      • Absence of active managerial control observed during the inspeciton. The person in charge is not monitoring handwashing activiies of employees, maintaining temperatures of TCS foods during cold holding and could not adequately communicate food allergens.
      • Observed an uncovered disposable beverage container stored on a shelf over the reach in cooler #2 which has food containers stored on it.
      • Observed a food employee wash their hands in the three compartment sink.
      • Observed food employee lather their hands under running water in the kitchen handsink. Hands were not adequately washed due to handsoap being flushed down the drain during handwashing procedures.
      • Observed food employee arriving at work to start their shift and handle a clean knife without washing their hands.
      • Absence of hand drying provision was noted at the front area hand sink.
      • Observed a ladle in the only handsink in the kitchen.
      • Observed absence of handsoap at the only handsink in the kitchen
      • Observed raw animal derived foods stored over ready to eat bread and packaged cheese in the 2 door upright cooler.
      • Observed the interior door of the two microwaves in the kitchen soiled.
      • Observed 2 plate, one frying pan and the food contact surfaces of the meat slicer soiled.
      • Observed the following surfaces of equipment in contact with Non-TCS foods soiled: the only can opener in the facility, upsplash of the juice machine and upsplash of the coffee brewer.
      • The following TCS foods were observed cold holding at improper temperatures: Shell eggs at room temperature (68°F) pulled from the walk in cooler at 8:20) Prep cooler- ground beef stuffed grape leaves (48°F), sliced tomatoes (51°F), Tzatiki sauce (47°F), Sliced cheese (48°F), Shredded cheese (49°F), cut lettuce (46°F), Feta cheese (48°F), sliced ham(45°F), sliced turkey (45°F), sliced corned beef (45°F), tuna salad (48°F). 2 door reach in cooler- raw scallops (45°F). The person in charge reported the TCS foods have been in the unit overnight.
      • Observed Chlorine sanitizer bucket located at the cook line exceeding 200 ppm.
      • Observed wrapped raw chicken stored in a plastic bucket on the floor near the 2 door reach in cooler. The person in charge reported it was going to be cooked.
      • Absence of ambient air thermometer in the 2 door reach in cooler and a working ambient air thermometer in the prep cooler.
      • Observed harboraging conditions in the food establishment including significant build-up of debris on the floors under and inside food contact equipment, soiled food contact equipment on the floor near the three compartment sink, exterior doors leading into the establishment are not tight-fitting or sealed, observed the exterior door leading into the food establishment opened upon arrival.
      • Observed presence of pests in the food establishment. Observed two flies on food in the 2 door reach in cooler, two flies in the kitchen and one crawling insect on the wall near lobby of the food establishment.
      • Observed the front door and the exterior walk in cooler are not tight-fitting, observed gaps along the bottom of the door.
      • Observed two flies on uncovered and packaged food which were not fully sealed in the 2 door reach in cooler.
      • Observed the following equipment in poor repair- Gaskets in the upright cooler and freezer are stripping, the left side door of the upright freezer is lifting away from frame, water at base of the prep cooler and 2 door reach in cooler which could indicate a cold holding capacity issue due cold holding temperature violations observed.
      • The person in charge could not provide a Chlorine testing kit.
      • Accumulation of debris was noted on the following non-food contact surfaces: interior of the fryer, interior and exterior of all refrigeration equipment, top of the dishwasher.
      • Significant accumulation of debris was noted in the following areas: The floors and walls throughout the facility, especially under cooking and warewashing equipment and interior of the mop sink, observed food debris inside the basin.
      • Observed the following in poor repair- blown of ceiling lights in the kitchen, flooring in the warewashing area has become rusted in sections.
