Issaquah School District: COVID-19 Vaccine Pop-Up Clinic For Issaquah School District Students Age 12 And Up

We will host a COVID-19 Pfizer vaccine clinic for our students, in partnership with Costco Pharmacies. Students must be age 12 or older as of May 20, 2021 to be eligible for this vaccine clinic.

  • Thursday, May 20, 8 a.m. – 12 p.m.
  • Friday, May 21, 8 a.m. – 12 p.m.
  • Tuesday, June 8, 8 a.m. - 12 p.m.
  • Wednesday, June 9, 12 p.m. - 4 p.m.

The second dose appointment will be automatically scheduled after receiving the first dose.

Please park in the main lot and come to the entrance. You will be directed to the gym from there.

All vaccine recipients must have a completed consent form. Students under 18 must have the form completed and signed by a parent or guardian. You can download and print consent forms here:

If a parent or guardian is present at the clinic with their student, consent forms will be available to be filled out and signed.

If a parent or guardian is not present with their student, they must print, fill out, and sign the consent form and send it to the clinic with the student.

Students who miss class to be vaccinated at the clinic will be marked as an excused absence.

To lower the risk of the spread of COVID-19, upon arrival, we will take your temperature and ask you questions about COVID-19 symptoms, exposures, and travel.
