Matthew Melvin Gibbs, 54, of Richard Boulevard, is charged with driving under the influence.
Police said Gibbs hit a power pole and a tree in a July accident. Blood was drawn at the hospital and tested, police said.
Gibbs' bond was set at $5,000 for an appearance in Derby Superior Court.
